Permanent Items, often referred to as 'PERMs,' are items that can be spawned in from your inventory, and are usable at any time. Typically, these PERM items are found in Catalog Crates, but they can also be obtained in other ways; mainly, event grants or developer distributions. Permanent items or "perms" have been widely regarded as unbalanced.

- Milestone rewards idea has been officially scrapped due to the data-losses & modifications caused by a group of exploiters. When asked, Edd_E stated that most of the items were purely in concept form and did not have any actual implementation yet. The only exception of this was the genie lamp which had actual functionality, this was proven when a group of exploiters (the same ones from before) found a method to spawn the Genie in.
- Alpha Permanent items (Hatchet, Guitar, Jetpack, M1911) were available in loot crates during alpha but were removed upon the transition into beta.
- Some players might have had their data affected by exploiters during that kind of incidents. That can be seen by the developers due to PenisSize value being modfied.
- Some Players have obtained specialized permanent items by being the first to report game-breaking bugs, glitches and vulnerabilities.
- T_sai is known to own a permanent Famas & permanent Static Jetpack for reporting duping methods, but is currently banned.

- PsychoBuckett is known to have a permanent Propeller Hat, Famas, and Recaller.
- JumpyBageI is known to own the permanent Famas, and has recently received [PERM] Evermore Dagger.
- FindNoob007 and plainizm are known to have recently obtained a permanent Evermore Dagger for reporting the persistence of a previously patched data changer during one of the game's exploiting incidents.
- It has been confirmed by Edd_E in the game's Official Discord Server that limited-time event crates have a pity system which grants player a 100% chance permanent item after a certain amount of unsuccessful attempts of trying to get it from the crate.
